Overview

Migraine is a neurological syndrome characterized by altered bodily experiences, painful headaches, and nausea. It is a common condition which affects women more frequently than it does men.

The typical migraine headache is one-sided and pulsating, lasting 4 to 72 hours. Accompanying complaints are nausea and vomiting, and a heightened sensitivity to bright lights (photophobia) and noise (hyperacusis). Approximately one third of people who experience migraine get a preceding aura, in which a patient may sense a strange light or unpleasant smell. Patients often describe triggers they feel precipitate an episode of migraine, such as certain foods and beverages (like chocolate or alcohol), stress or menstruation. In some migraine types there are typical features but the headache remains absent, and in children abdominal pain may be a prominent feature.

Although the exact cause of migraine remains unknown, the most widespread theory is that it is a disorder of the serotonergic control system. Genetic factors may also contribute. Studies on twins show that genes have a 60 to 65% influence on the development of migraine. Fluctuating hormone levels show a relation to migraine in several ways: three quarters of adult migraine patients are female while migraine affects approximately equal numbers of boys and girls before puberty,[citation needed] and migraine is known to disappear during pregnancy in a substantial number of sufferers.

The treatment of migraine begins with simple painkillers for headache and anti-emetics for nausea, and avoidance of triggers if present. Specific anti-migraine drugs can be used to treat migraine. If the condition is severe and frequent enough, preventative drugs might be considered.

The word migraine is French in origin and comes from the Greek hemicrania, as does the Old English term megrim. Literally, hemicrania means "half (the) head".

As a result of my clinical experience I have developed an approach, which not only determines if disorders of your neck are likely to be the source of your headache or migraine symptoms, but can also identify the spinal segments at fault. This manual diagnostic accuracy is unparalleled and significantly increases the chances of a successful outcome because treatment can then be directed at specific and relevant segments.

All about Migraine

Migraine is a condition of the brain characterized by headaches, nausea and changed bodily perceptions. It more frequently happens in women than in men. The word migraine is from a Greek term "hemicrania" meaning half skull and the Old English word "megrim" which means severe headache.

A typical headache caused by this condition is pulsating and unilateral. It lasts from 4 up to 72 hours. The symptoms include, vomiting, nausea, sensitivity to light or photophobia and sensitivity to noise otherwise known as hyperacusis. According to studies, about a third of the people suffering from migraine headache identify and aura which is olfactory and visual that announces the headache.

Causes of Migraine

Initial treatments are analgesics and anti-emetics. Analgesics are for the headache while anti-emetics are for avoiding triggering conditions and nausea. Its primary cause is still unknown. An accepted theory however is a malfunction of the "serotonergic control system" for PET scans verified the aura that corresponds with the transmission of the cortical depression. This results to increased level of blood flow, which is over 300 percent greater than that of the baseline.

There are migraine variants that start off in the brainstem featuring cellular transport dysfunction of potassium and calcium ions. Some others are genetically predisposed. Studies show that twins have a 65 percent chance of genetic influence leading with increasing susceptibility to migraine headache.

Irregular hormone levels could also lead to migraine. Thus we see many women feeling low, heavy, and dizzy just before their period. Pregnant women are also susceptible to severe headaches.

Preventive Treatments for Migraine

Preventive treatment is also known as prophylactic. It may or may not be a significant component of managing migraines. The method involves taking nutritional supplements and certain drugs.

A patient might have to alter his lifestyle to prevent foods or any activity that leads to migraines. The main goal is to significantly reduce the pain, duration and frequency of occurrence.

Migraine Medication

Below is a list of drug class prescriptions effective in curing this condition.

Beta Blockers
This drug class includes atenolol and propranolol. The Cochrane Collaboration held a meta-analysis of 9 crossover studies or controlled trials amounting to 668 patients. They found out that propranolol has a relative risk of treatment response.

Anti-Convulsants
This includes topiramate and valporic acid. Again, the Cochrane Collaboration did a meta-analysis of 10 crossover studies or controlled trials amounting to 1341 patients. They found out that anti-convulsants are twice as likely to experience greater decrease in frequency with anti-convulsants as compared to Beta Blockers and Placebo. Nevertheless, there were some concerns about gabapentin marketing.

Anti-Depressants
A type of anti-depressant is TCA or tricyclic antidepressants. Examples are amitriptyline along with SSRI or "selective serotonin reuptake inhibitors" like fluoxetine. According to Cochrane Collaboration"s meta-analysis, SSRI is not much effective like placebo.

Another study revealed that SSRI is beneficial or effective on patients suffering with tension headache and migraine. Nonetheless, there was not a separate report among the effect of SSRI on patients with only migraine issues.

A wide variety of pharmacological drugs are evaluated so as to know their efficacy in decreasing the severity or frequency of migraine attacks. Evaluated drugs include calcium antagonists, beta-blockers, neurostabilizers, NSAIDs or "nonsteroidal anti-inflammatory drugs, SSRIs and tricyclic antidepressants as well as other particular drug therapies.

Headaches affect a large proportion of us at one stage in our lives, although degrees will vary. Headaches also affect children, particularly before puberty. Unlike adults, boys and girls are affected to equal levels. However, women will experience more severe headache than men. (6% of men and 18% of women)



How can we treat headaches?

Herbal Home Remedy for Headache



Headaches can develop for a number of reasons usually they can be related to some obvious cause such as sinusitis, eyestrain, fatigue or tension. The majority of the headaches are due to stress or worries with muscle spasms in the neck leading to headache.

Try these home remedy for headache:

* Mix a quarter teaspoon of clove powder in one teaspoon of cinnamon oil. Apply mixture to the location of pain.
* Take a few drops of ghee (clarified butter) on your palm. Mix 3 or 4 pieces of saffron in it, and rub it for two minutes. The saffron does not dissolve in the ghee, but its qualities are absorbed. Remove saffron and put two drops of this ghee in each nostril. Dosage: Follow procedure twice a day.
* Mix 1 tablespoon of milk and half teaspoon of ginger powder and put one or Dosage; two drops in each nostril twice a day.
* Cut a lime in half, rub on forehead.
* In case of headaches caused by sun radiations, flowers of henna are found effective. Rub the henna flowers in vinegar and apply it all over your head. This home remedy will give instant relief.
* For headaches, eat a banana every time the linger headaches appear.
* Do not skip meals, especially for migraine sufferers.
* When headache is caused by cold winds, cinnamon works best in curing headache. Make a paste of cinnamon by mixing in water and apply it all over your forehead.
* Use heat: a warm washcloth or a hot water bottle can ease pain
* Lying down in a dark, silent room may also relieve headaches. This can be combined with massage or pressure application to the bridge of the nose. Also, applying a damp, cool cloth when lying down should help alleviate the pain.

How herbs and Natural Remedy work for Headache?



Herbs contain small quantities of chemicals, compared to modern pharmaceutical products which extract or synthesize one chemical in much larger amounts. This means there is no danger of sudden physiological changes which cause side-effects.

For example, valarian appears to improve the quality of your sleep as well as helping you to dose off and doesn't cause a sluggish feeling in the morning, because the chemicals it contains are in very small amounts and don't last long in the body. Recently much has been made of a research trial which showed that a St John's wort preparation made liver enzymes more active, which reduced the effect of other drugs taken at the same time because they were metabolized before reaching their target.

The St John's wort preparation used was standardized to contain a larger amount of one constituent - hypercin - than all the others. Not only has hypericin failed to show antidepressant activity on its own in repeated trials, but another ingredient - hyperforin, has been shown to counterbalance hypercin in its effect on other drugs and doctors in Germany continue to prescribe it as a favored anti-depressant.
Nautral Headache remedy using Cinnamon

Cinnamon is useful in headaches caused by exposure to cold air. A fine paste of this spice should be prepared by mixing it with water and it should be applied over the temples and forehead to obtain relief.

Similar bad publicity surrounds liquorice, where glycyrrhizin is thought to raise blood pressure
, but in fact dozens of other constituents act to lower it, in particular by diuresis (elimination of water). There are many more examples of this sort of balanced action. Where two or more constituents act together to create the same effect this is known as synergy. Types of headache and their treatment


Headache


Reports suggest that at least 40% people in the UK suffer headache at some point to the extent that can affect the quality of their life. It is generally defined as a pain in and around the head. Some types of headaches are discussed below briefly.



What are tension headaches?


The most common type of headache suffered by the majority of people worldwide is due to tension-type. Tension headache are also known as stress headache or muscular headaches. If it occurs more frequently, then you should seek treatment. However, occasional headaches are common to occur at least once or twice in a month. During the times of excess stress, headaches can be even more frequent. What is Sinus headache?


If you have a deep and constant pain in your forehead, cheekbones, or the bridge of your nose, then these are considered as symptoms of Sinus headache. Some other symptoms of Sinus headaches are runny nose, fever, and facial swelling.


Treatment of headaches


Irrespective of its type, headaches can greatly affect your sleep and life. It can lead to fatigue, tiredness, and even stress. Anaesthetising Neck Structures Cease the Hemiplegic Aura of Migraine

In a recent report anaesthetising selected neck structures ceased the hemiplegic aura in a two migraine patients.

This patients" auras comprised slight weakness, tingling and or numbness involving one side of the body, and face. Within 5 minutes of injecting a local anaesthetic the auras ceased and were not followed by their usual pain states.

I have on two occasions prevented auras from developing, in fact they ceased within 5 minutes after mobilising the upper cervical spine - and furthermore the usual pain did not occur. My experience and what is reported in this case report suggests that abnormal information from a neck disorder could be sensitising the brainstem and suppressing it prevents the migraine occurring.

I have mentioned temporary reproduction of familiar pain as a key diagnostic sign for cervicogenic headache; in a significant proportion of patients suffering migraine with aura it is also possible to temporarily reproduce their auras with subsequent lessening (as the examination technique is sustained) when examining the upper neck - once again this is a good sign and confirms cervicogenic factors as the source of sensitisation.

It is not necessary for confirmation to have invasive (nerve block) procedures ...

Cervicogenic neck Headache and Migraine or the Same Condition?

Why is it that if treatment of the neck provides relief that it is assumed that cervicogenic co-exists with migraine or that it is a misdiagnosed migraine? Why can"t it be that cervicogenic factors are instrumental in the migraine process?

Clearly when patients with "migraine" who have not responded to recognised migraine treatments, achieve substantial pain relief to numbing of the greater occipital nerve, there can be only one conclusion ....

Headache, Migraine and Facial Surgery

In survey of 75 patients who had undergone surgery for facial pain all had ongoing symptoms. The researchers concluded that the causes of symptoms were migraine, cluster headache, paroxysmal hemicrania and tension headache, and that sinus surgery in the large majority of patients presenting with facial pain should avoid surgery; that all surgeons when assessing facial pain should be aware of non-sinuonasal diagnoses.

An enlightened ENT Specialist refers patients to me to determine if facial pain is referred from disorders of the upper neck - and more often it is! Temporary reproduction and easing of familiar facial pain when upper neck structures are gently stressed confirms this in the large majority of patients with facial pain.

ver the past 15 years I have developed a series of techniques, which, by way of temporary reproduction of headache and easing of the headache as a technique is sustained, confirm that a neck disorder is the cause of or a significant factor in the mechanism of the headache or migraine - this a key diagnostic criterion for cervicogenic or neck involvement in headache according to the International Headache Society - importantly for the disorder to be related to the headache or migraine process the headache has to ease as the technique is maintained. If both reproduction and lessening are not possible then the neck may not be the source of the headache or migraine. Furthermore my experience has shown that if the techniques are performed in a specific manner it is possible to determine which spinal segment is the cause of or contributing significantly to headache and migraine. Having determined which spinal segment (or segments - there may be more than one) is involved then this significantly increases the chance of the treatment being successful because treatment can be directed at specific, relevant spinal segments.

What Does Blocking The Greater Occipital Nerve Do?

I have been mentioning the research which shows that by injecting and anaesthetising the greater occipital nerve the migraine process can be halted ... so what does this do?

Anaesthetising the greater occipital nerve prevents information from (neck) structures supplied by the nerve from entering into the spinal cord (and the brainstem). The effect of this on migraine and headache symptoms suggests that this information being carried by the nerve was sensitising the brainstem and that by preventing it from reaching the brainstem, the brainstem is no longer sensitised.

This then is considered a diagnostic tool to confirm cervicogenic (neck) disorders in headache and migraine.

Although Physiotherapy, Chiropractic and Osteopathy are different disciplines, we are supporters of the idea that headache and migraine sufferers, no matter what their diagnoses, can be helped to live their lives more effectively through appropriate neck treatment. However, because treatment of the neck does not fit the medical model of headache and migraine, the model has demonstrated little interest in exploring this as an option. It is essential that all factors, which have the potential to sensitise the brainstem, be investigated equally. Currently this is not the situation - the neck is largely disregarded.
